For all healthcare related referrals, please have your Family Physician send a referral using the links. 

 

This form can be used for referrals to Southern Alberta Surgical Center, Rideau Clinic or the Breast Health Clinic at SASC. All referrals are reviewed thoroughly by one of our surgeons or physicians and triaged appropriately. Please note we will provide advice on management for our referring physicians to initiate while waiting for surgical care. It is necessary to implement this given the delays and restraints in our current Canadian health care system.
